I wouldn't think you would have a problem. She is infact your mother and I will assume that you have the same last name as your mother.
When you have your car insured it will most likely be in one parents name but seeing they are married, the law carries that it is good with both of them. If you follow what im saying.

I don't know where you live but I can tell you the laws in Florida. I had to sign a certificate for each of my girls when they got their learners permits. That certificate that I signed here said that I was assuming any and all financial responsibility if my daughter had an accident while driving. Even if she was in a stolen car and I had no knowledge of it, because i was getting her the learners permit I was assuming these responsibilities.
As far as the driving with supervision goes. Here, you have to have a licensed driver over the age of 18 in the front seat with you. It does not have to be your mother or father as long as it is a legally licensed driver over 18 they can be considered your supervision.

You're good to go. The certificate is just a notification to the state that you will have supervision in the vehicle with you. Your mother did not need to sign it. The law simply requires that you be supervised by a legal adult with a valid driver's license. So, your dad, mom, grandparent, or even a family friend, as long as they are an adult and have a valid driver's license, can be the "supervisor" in the car.

Each state's laws are different, but generally "supervision" means someone of a minimum age, with a valid license, and a minimum amount of experience themselves.
Your mother almost certainly qualifies. It's probably in the material they gave you with the license.

Yes,she can go with you. The certificate isstating that a parent or guardian may go with you when you are behind the wheel of that vehicle.

Should be fine. Usually it's just someone over 18, or 21 depending on the state. I would assume your mother meets either of those ages...

Well I know for as far as the state of Ohio goes its anyone over the age of 21 that has to be with you while you drive. I also don't doubt that it definitely includes your mom.
