Yes I have seen a dog die from eating chocolate (not mine however)
Chocolate contains theobromine. A naturally occurring stimulant found in the cocoa bean,theobromine increases urination and affects the central nervous system as well as heart muscle. While amounts vary by type of chocolate, it's the theobromine that is poisonous to dogs.
What most fail to realize is the accumulative effect of theobromine. So while it appears as if a small amount of chocolate has no adverse effect, over time those small amounts add up to a fatal dose.

I have seen Chetco mention more than once that her pug died after eating a few M&M's and I've also seen a couple of vet techs mention that they had dogs come into thre vet's office and the dog died as a result of choclate poisoning.
So, although, it's never happened to any of my dogs, which don't eat it anyway, unfortunately, it can and does happen.

No, thank God, but it is possible. Dark chocolate is extremely toxic to dogs, where as most milk chocolates will probably just result in vomiting.

dogs do not have the enzyme to digest chocolate . about 4 oz. is enough to kill a small dog .

IIRC they have to eat quite a bit to die. Mine have gotten into chocolate several times, and it gives them the runs something feirce

yes the vet said it burnt a hole through her stomach and she bleed internaly and drownd from the inside.
