There are no set guidelines for something like this.

Find a new doctor.

First thing I would do would be to go back to your current doc and ask who you are suppose to seek out to sign your forms. Your current doc should be able to at least direct you in this regard.

Another possibility: If you have seen another doctor prior to seeing the doctor you are currently seeing, who does know your background, you have two options: make an appointment with that doctor to have the forms signed, or ask your current doc if you sign release forms to have your background records sent to him/her, will he/she sign the forms after he/she reviews them.

If you have seen many docs in the past, you will need to sign a release form at your current doc's office so that one doctor has all your info. Confirm with your current doc that after he/she reviews the information, he/she will consider the documents you need signed.

If all this fails, explan the situation to the department of motor vehicles and ask them what you should do. They should be able to direct you since they know their policy and they likely have dealt with this problem before.

did you even think about seeking the help of a mental health advocate like I suggested in one of your other 10 posts about this same situation. They could help you a heck of a lot more than us especially since it is obvious you don't like any of our answers.

A mental health advocate is there to help you. They won't cause you any more turmoil. You are either afraid to seek help for some reason or you are so lost in your own world, you don't uderstand all that is going on around you.

Please seek help in the real world.
