It's hard to reliably say with any degree of accuracy for undeveloped nations. Those who have to hunt for food are undoubtedly much faster than those who can drive-up to a McDonalds.
By the time you weed out the very young, the very old, and the many obese, it's a precious few with that blend of speed and endurance.

A five minute mile is definitely not world class but it is pretty darn fast if you are talking five minutes flat. When you consider everyone in the world it is a very small percentage. No one in there 70s and beyond could do it. Maybe one out of a million people in their 50s and 60s could. Obviously the age group from 15 to 35 would have the higest percentage of people that could. But at the local high school which has around 1000 young healthy kids I bet maybe 3 kids could run 5 flat (that is .3%) Once one is out of high school only people very serious about their fitness would be able to do it. In the Portland Metro area there are probably a couple of million people. Being pretty familiar with the running scene I would say there are 50 to 100 people who could run 5 flat. So my answer would be .05 to .1 percent at the best and probably even less then that when looking at the whole world.

I would guess that about 33% can run a 5 minute mile. On my high school track team right now, we have 5 runners that can. My team is nothing close to being State Champion level, but a lot of us can do it. At Invitational meets, I've seen as many as 40 athletes get 5 minutes or better. Also, I think a lot of runners can do it, if they were to run for time and not for exercise. Honestly howmany people run each day?

Let's think about this one -- 5 minutes is pretty fast, even if we are talking for only a mile. Remember -- we have to throw out just about everyone who is not pretty close to top shape -- plus most who are too young or too old. Then, there's a gender bias, since men have been faster than woman overall (although certainly not on a case-by-case basis.) I'd be surprised if 1 of 1,000 could run a 5-minute mile.

on my h.s. track team way back when, at least six of us could run that fast, about 4 of whom were seniors. that's four people out of about 300 in my graduating class. however, i'm guessing that number decreases once people go to college or finish high school. a five minute mile won't get you any scholarships, so your likeliness of setting a personal record after h.s. is pretty small. so i'd say the highest ratio per age group would be in the 18-20 age group. and using my graduating class as an example, i'd say anywhere from 1-2 percent.
