Take a picture of the 2 of you and buy a very nice metal frame (one you can engrave on). Engrave your names on the top...such as Amy and Ashlie
then on the bottom of the frame engrave:
Maid of Honor for a day...sisters and best friends for life.
After the wedding she can put a wedding photo of the 2 of you. You could put your wedding date on it too if ya want. Sisters are special and you have a great one. Best of luck to you and I wish much happiness.

I agree with a piece of jewelry that maybe has her birthstone. Something very special she can wear to the wedding. Arranging a babysitter so she can get a way for a weekend would also be great. Since she just got out of the hospital, how about going over to her house and make several dinners for her? Freeze them so all she has to do is pop them in the oven.

A popular maid of honor present is jewelry to wear at the ceremony, so you could surprise her with extra-extravagent piece of jewelry, if you can afford it - diamond earring or pendant, or whatever stone matches the dress or her birthstone. And write her some sweet line about while these may be precious stones, she is even more precious/valuable than any diamond.
Or send her on a "honeymoon" of her own - pay for a nice trip for her and her significant other or kids or even just by herself to recharge and enjoy herself.

How about putting together an album or even a dvd to music of pictures of the 2 of you from birth. I think a day at the spa would be nice to. Go together before the wedding just the 2 of you.

Oh, what a great sister (both of you!) It might sound a bit generic but perhaps a nice piece of jewelry, a ring or necklace, perhaps with a martching one for yourself. Or how about a day at the spa with massage and treatments, you could share the day together and organise childcare for her. make sure you tell the massage people you are pregnant before, they will massage differently. Congratulations on the baby.

I was the MOH for my best friend this past summer and her husband actually got her and I matching rings. It was so sweet of him to do this for me. He actually presented them to us at her shower during the Old, New,Borrowed, and Blue game, I had set it up with him that when she opened the gift it would be her "something Blue" which she didnt have for the wedding day and since her birthstone is aquamarine he got her a beautiful ring to wear on her right hand. When she opened the box though, there was an additional box in it for me with a matching ring (we are two days apart in birthdays so it was my birthstone as well) It was such a wonderfully sweet gesture and I was so touched that he would do something like that for me, and I came to find out that they had been shopping one day and they had come across this ring and she said she wanted to get it for me herself. So that he had remembered this and gone out on his own and picked these rings up for both of us was extra special for me.
I've gotten many gifts as an attendant, but none that special. And she and I both wore our rings on her wedding day and took a special picture of our hands together. It was great.

I know it may sound crazy, but if she is raising two little ones of her own then maybe she needs some financial help.
Other than that my maid of honor was my best friend and my sons god mother... I couldnt think of anything real special so we got matching tats....
We each got the chinese symbol for friendship.

That is so nice of her! Get her a nice spa treatment and offer to babysit her kids. Or get a spa treatment for the both of you so you can tell her how much you appreciate her help in everything.
Even better would be to maybe take care of her medical bills.

Matching rings that like sort of resemble friendship rings showing how you two will be together forever. That would be sweet, maybe with each other's birthstones on it and has an engraving inside it or something.

Why not get her a necklace with her birthstone in it.
But also write a letter to her telling her how special she is, and thank her for all she has done for you. That will mean more than anything you could buy her.
