Ibuprofen is an NSAID = NonSteroidal Anti-inflammatory Drug. Other over-the-counter drugs in this class include aspirin and naproxyn.

NSAIDs work by inhibiting cyclooxygenase, and enzyme involved in producing inflammatory chemicals messengers. These "chemical messengers" produce pain and inflammation. Therefore, NSAIDs, like ibuprofen, not only reduce pain, but they also reduce inflammation and can do things like reduce swelling and stiffness in an injured muscle or in a joint with arthritis.

The products of cyclooxygenase are also involved in platelet clotting, which explains why aspirin can help prevent heart attacks and strokes.

Cyclooxygenase also produces compounds that help the stomach protect itself from acid. That is why long-term continuous use of NSAIDs is known to cause ulcers and bleeding.

The mechanism of Tylenol is less clear, but it seems to act in the central nervous system to reduce pain. It does not control inflammation and does not affect blood clotting.

Both Tylenol and the NSAIDs do control fever.

Summary:
NSAIDS=pain control (analgesia)
anti-inflammatory
control fever (anti-pyretic)
reduce blood clotting
long-term use can cause ulcer and GI bleeding

Tylenol=pain control
anti-pyretic
SAFE for the stomach
