Can I just like go to the hospital from which I got my physical a while ago and ask for a copy of it? Or do I need my parents consult or presence, since I am under 18?

I live in MA, USA.

Not sure since you are a minor. Certainly if you are 18 or over, you have a right to a copy of your records.

The best thing to do is to call the hospital and speak to your doctor or someone in medical records. Medical records personnel can tell you the local laws and regulations.

If you are unsure who to contact, simply call the main operator number for the hospital and ask to speak to someone regarding obtaining a copy of your records, they'll transfer you to the right person.

Good luck

You should be able to go to the medical records department of the hospital that you had your physical and request a copy of it. I have had several operations and the hospital that I had them at gave me a copy of all of them when I asked for them. it took a couple of days for them to copy all the information.

Absolutely, you are entitled to any medical records about yourself. You may have to pay a copying fee though, I did. It was like 47cents per page I think

the information in your medical record is yours for the asking. They cannot refuse to give you a copy, but they can charge you for the cost of copying it.

You will need a parent with you to sign for them since you are a minor and have no identification.
