Is this your only income? Did you receive a W2 or a 1099?

If it was a W2 you are not required to file.

If it was a 1099 then you are required to file a tax return. The income would be considered self employment income and would go on a Schedule C and the self employment tax would be calculated on the Schedule SE. Don't forget to subtract out any expenses that you had to pay out of your own pocket that you were not reimbursed for. The numbers are the brought over to the 1040.

You are required to report the income - whether you received a W-2, a 1099, or no form at all. It is treated as 'self-employment' income and added to your other income for 'income tax' purposes. Self-employment (social security & medicare) taxes are also due (15.3%). Use schedule 'C' or 'C-EZ' and schedule 'SE' to figure tax and report the income. Even if you have no other income, it MUST be reported (if it is over $400.00). If you are considered a 'church employee', use the Long schedule SE.
