Depends on the hotel. Usually, it will be a bigger room, perhaps with a bigger bed or better facilities (TV, minibar, etc). The only way of checking exactly what the difference is would be to call the hotel.

It depends on the hotel; each have different amenities. But I would expect a Premium room would be larger than a Standard room, might or might not have a view, and would probably have some extra features like a nicer bathroom (big tub or jacuzzi maybe?), a small sofa or chairs in a sitting area separate from the sleeping area, maybe a larger bed, or more closet space, or a small kitchenette-type area with sink and mini-fridge. I'd also expect a Premium room to be further away from the elevators and ice machines and on a higher floor than a Standard room, so it is quieter.

Premium can mean almost anything as long as it is "nicer" or "better" than the standard room.

depends on the hotel, you may be on a higher floor than a view room, you may get a fridge which a view room doesnt or you might get a minibar, it may even be a slightly bigger etc...
you should be able to find out looking at reviews on tripadvisor.com

I think you get a phone in the bath room, trouser press, bath robes and a full mini fridge!

You might just get clean bed linen and a carpet that isn't too stained.
