You don't have to give the Doc a diagnosis, just your symptoms. Explain what feels wrong; when it happens; how it affects your functioning; what you've tried to do about it yourself; how long it's been going on; and how severe it feels.
Specifically about anxiety, there might be sweaty palms and underarms, sleeplessness, low appetite, social shyness, difficulty concentrating or remembering, excessive use of alcohol, hyperventilation, or even feelings of panic. Try to describe your inner feelings, which there are no objective tests or X-rays for, but also any physical, objective and possibly measurable syptoms.
Then your doctor will take over and ask questions, to guide you along.

I would say just be prepared with a list of symptoms/concerns. I wouldn't think there would be anything wrong with saying what you think might be the issue, but also listen to what the doc has to say. Doctors know these disorders, but you know yourself, and that shouldn't be discounted. A good doc will appreciate your awareness of your own problems, and hopefully between the two of you, a solution can be found. Good luck!

It's especially dumb to tell your doctor what's wrong with you if it's a psychological disorder like an anxiety one, and your doctor is a family doctor and knows little about psychiatry. What you should do is get a referral to a specialist who can actually help you.
Also, there's a great cure for anxiety, talking to someone about it! It's one of those old home remedies!

Your Dr. is most likely going to be more than happy to prescribe a little pill to fix things up a bit.
Or you could help yourself...
I spoke to a Psychologist/Herbalist/Naturopathic Dr. who used to buy his herbs in the shop where I apprenticed, and I asked him, because I hear so many people with the same issues, if he could just give me a few words of wisdom for health seekers.
He said,
1) Eliminate refined sugar, caffeine, and alcohol from the diet.
2) Eat a healthy, whole foods diet.
3) walk 20 minutes every day.
4) Drink plenty of GOOD water.
5) Do something for somebody else on a regular basis.
If anxiety persists, try Hypericum (St. John's Wort) 2 caps 3X daily.
It's worked for a lot of my clients!

i knew somethin was wrong with me i was always freakin out while out with people that were my friends large crowds would bother me so what i did was i wrote down everytime i thought somethin was a little off when i went to see him i found out that i have a major anxiety problem he put me on meds and two years and still goin i am a lot better i can hangout with my friends i can even meet knew people if you think there is something wrong there probably is you know you better than any doctor will just go with facts trust me things will work out

Yes, they do. I sure wouldn't start out by just giving him your diagnosis. That would probably just put him on alert for the possibility of hypochondriasis or something like "medical student syndrome." Just let him know about your symptoms without actually telling him you think you have a disorder. Leave that part up to him to decide.
