I have had 3 patients with mononucleosis in the last month. They are having the same symptoms as your friend.
Whatever he has sounds viral. It should not be going on for 3 weeks though.
I am not a physician, but nurse in a hospital. If I were a physician, I would order the following
1. Urine culture
2. Blood cultures
3. MonoSpot
4. CBC and MPB to see if his cell counts and chemistry is ok
5. Strep throat culture
Of course he could just have some virus looming which takes awhile to shake, but 3 weeks is a long time.

many doctors are not literate in things such as Chronic Lyme Check out on line the Lyme association .. Lyme can only be detected from exposure to perhaps 3 weeks then it is no longer able to be detected by a regular doctor because the body stops making antibodies and stops fighting the infection. Then the Lyme goes into the body undetectable unless labs are sent to a place that tests for the DNA of the Lyme bacteria . It goes into chronic stages where sufferers go with no help. Seek out a Lyme specialist sounds like that maybe an underlying thing here.
