if you're like most americans, you'd contact a lawyer first.

It depends on the nature of the complaint, always to the doctor treating you first (in writing), if not satisfied move up to the Hospital management team. Most hospitals have a patient guidance service to advise you of what to do.

I would think it would be to the hospital the patient was at. Get the name of the Chief Executive and address the complaint directly to them.

Ring the hospital in which the incident happened, explain you need to send a letter of complaint and need an address. They wont bat an eyelid its an everyday occurrence!

you would have to get in touch with the hospital where the patient was treated. go on to their website and you might find a link there where you can complain. if not, there should be details about the people in charge on the hospital boards where you can write to them. Hope this helps.
