Externally so many reasons, Draupadi, Dhritarashtra, Duryodhana etc. . . ultimately it was the plan of Lord Sri Krishna to reduce the burden of earth by making the various unrighteous kings to assemble together in the battlefield of Kuruksetra.

Lord Sr Krishna states in Bhagavad-gita 4.7-8,
Whenever and wherever there is a decline in religious practice, O descendant of Bharata, and a predominant rise of irreligion—at that time I descend Myself. To deliver the pious and to annihilate the miscreants, as well as to reestablish the principles of religion, I Myself appear, millennium after millennium.

Undoubtedly Duryodhana. Drutharastra, had Vidura as his advisor. He was contemplating and attentive
But Duryodhana, a personification of Kali, was stubborn. He had , in his mind, invincible Bhishama on his side, whom, Lord Krishna, also could not do any thing.

For Mahabharata war , the above 3 are not responsible. Lord Krishna is the main planned of this war. If u have read Mahabharata book u could really guess it. Krishna forced Shaguni to call PANJA PANDAVAS for playing that game. So Krishna planned that surely this will end in the war. Because GOWRAVAS willnot follow DHARMAM ( means rules which a man should follow for living) so after 14 years they willnot really replay to PANJA PANDAVAS. So Lord Krishna is the main planner and he played the FATE game which seeded MahaBharatha war.

The way I see it, things were already smoldering in terms of the relations between the Pandavas and Kauravas. The incident with the disrobing and humiliation of Draupadi was the catalyst that set things in motion.
