go to the post office, get an application.
i suggest getting 2 or 3 just in case you misspell something
and confirm all the information. it'll ask for your parent's full names and birth places and dates
i can't remember if mine asked for their ss#'s

gather all your identification... your birth certificate, your social security card, and your state issued id card or driver's license. bring anything else that's listed on the application that you can reasonably supply, just to be thorough...

then find a place to get a passport photo
you have to pay for them no matter where you go
usually places like walgreens, cvs, rite aid, or eckerd
some post offices will do it for you too, for a fee

then there's the fee.
according to my application (i haven't submitted mine yet)
it's about $97 for standard processing
and $157 for rush (3 day) processing

then bring the completeed application, the photos, all the requested identification, and the fee to the post office.

2 passport size photos signed by a jp or other approprate person, fill in the pass port form/document, birth certificate to show proof of nationality and the correct fee for processing the documentation.

go to the post office where they have a passport office and there u will find a check list of everything you need ... good luck

also go on line 2 they should have a list on what documents they accept
