It's a ratio between the primary and secondary windings. If you want to step the voltage up, from 12 to 20,000 as in an ignition coil there will be about 1660 times more windings in the secondary as the primary.

If you're stepping down the voltage, the reverse is true.

To answer that we need to know what the ration between the primary and secondary is and how many turns are on the primary. Assuming it doesn't saturate.
