I am usually so good about keeping up with all important paperwork but Ive been losing my mind lately and cant seem to find my daughters shot records. does the hospital normally print out another one for you or will she have to get them all over again? she is 18 months old and she got all her shots from the same hospital.

Depends on what state you are in. Usually you can tell the hospital or doctor and get another copy. They are required by law to keep track of what shots are given to whom. In Michigan where I am, you can call this number and get a state office that will print you out a copy. I've lost my 3 kids shot records more than once, so don't feel bad. If the same people give her the shots all the time, they will always have a record. You simply need one for school, traveling and such.

It depends on where you live but in MI, there's a system called MCIR Michigan Childhood Immunization Registry. When vaccinations are given, all drs offices/clinics/hospital enter the information and it's available to any other hospital/dr's office/clinic. It's really very nice. I'm sure most other states have similar things. Otherwise, you could just ask that place to write them all down the next time.

I've worked at a doctor's before...and the Medical Assistants or Nurses should be keeping record of all of the shots your daughter is receiving in her chart.

If you have a family doctor they would have them there. If you do not, the hospital would heve kept track of them for you.

So you do not lose them again, try getting them put into one small package so they fit in you wallet or purse. I keep mine right with my health card in my wallet and never lose it. I got a little 1in x 3in record book that has all my shots, dates, age and if I need it again.

In most cases, you can call the hospital and they will send you a replacement record. There might be a small fee involved. I think our doctor charges $10 for a replacement.

That's no problem and no reason to worry. They'll have all the records for you. You can make a special trip and have them fill it out again, or wait until her next check-up.

You can check where she received her shots. They keep medical records. You can also check your state registry. In my state parents have access to their child's shot records. Do a search on immunization registry + your state.

They should have it on record. You may have to pay a small fee to get a new copy of them. Does she have a regular pediatrician? If so, they would also have records.
