Is this for a college research project? I had this prompt for something recently too. Just write something like:

Dear HP Customer Service:

I recently purchased by laptop from Best Buy in Chicago, Illinois. It began having problems. Blah Blah Blah. I feel very disappointed that .... I will not be purchasing another HP product again. Yada yada

Put your address on the top upper right hand of the page.
Put the receiver's name just below that, but on the left hand side of the page. It's a pretty easy assignment.
