is it important to put a letter head on it , as in putting your name and address somewhere on the letter?? or is it ok just to write and sign it

Very Important:
I don’t know why you are leaving but PLEASE, do not put a reason of "personal reasons" if you plan to get unemployment.
Are you being forced out?
If you are, write something like: "I feel I have no other choice but to resign my position".

A resignation is proof of your intent to leave. You can choose whether or not to put your name and address it doesn’t matter. They have your address anyway. It will be put in your employee file and only be resourced again for the following reasons:

1. If you or the company is trying to take legal or formal action ( get unemployment) and need to determine why you left (if you say why) and if you gave a 2-week notice. (a must)

2. To see if you are rehireable.

3. Other technical stuff.

At this point you do not have to say "I’m leaving for personal reasons" and all that other stuff. You do not want to lock in on any statement, which may disqualify you for anything!
This includes writing stuff that is nasty ect.
Just keep it basic. Give the date you write it and the date of your last day. If you have vaca or other time or $ owed to you mention that you expect to receive it. (depends on state law whether you'll get it).

The reason I'm telling you this is because I resigned my job before I was fired because I was being forced out. I thought I could get another job right away. It's 6 months later and now I'm pissed! I just decided to file for unemployment before I lose my frickin house! YOU HAVE TO PROVE YOU left or you were going to be fired.
I don’t know if any of this stuff helps your situation but people don’t realize how a resignation letter can be your most useful ammunition if need be.
PS. make sure you give, fax and or email a copy to your boss AND Human Resources, the owner whoever. Just make sure you give it to 2 people. If your boss is pissed that you’re leaving, he/she may throw it out and say, "He didn’t give me anything".

Make sure that you are clearly identified and address it to the right person/office.
Be professional and SS&TTP (short sweet and to the point). Don't go on and on why you want to leave and don't use this as a ploy to get a raise or nark on a fellow employee.
Be prepared--some people will just say 'get out' when you give notice. It is imoportant that you are prepared to complete your projects, train a replacement or do your time so you can honestly say to your new employer that you won't leave him in the lurch and flake out like so many losers do.

Honestly, it is entirely up to you.

Your name should be sufficient at the top as an inside address (assuming that the HR or boss knows your address). Could be safe and include it.

Letterhead is not required. Don't for goodness sake use theirs.

I resigned once in an e-mail and this was very satisfying, not recommended generally, although mine was very unusual circumstance!

you can write and sign. I don't think you need to put an address, but it would be nice if it was typed. Good luck in the future!!
