Any thing that rides on top of rails can leave those rails. All it takes is the right circumstances.

Yes but it isn't likely. That system is well maintained and inspected. NYCTA isn't likely to let the conditions for a derailment arise. It's amazing how well it works, given as there are dozens of transit systems nationwide that almost never have derailments.
I've been in one, though. It was a railfan run using a 100 year old single-truck historic streetcar in San Francisco with a latent wheel gage problem that showed up (natch) in the curves above Dolores Park. We just went "Thumpalump" and the motorman stopped the car. Nobody was injured, it was just a surprise is all. Blocked the J Church line for an hour or two.

Of course it is possible. Although the chances would be slim, there is that small percentile that states the train can go off the rails. Any vehicle that rides on rails can go off track. Maglevs, High-Speed electric, Steam, etc. all have the capability of going off track. A mechanical issue within the locomotive or in one of the cars it is pulling could be the culprit of such an issue, or an issue with track (i.e distorted rail, material on the track).

yes but getting power if it is;nt using a pantograht set-up this is a standant gauge system in the US then again there is a third rail setup used in the mta also

No, they ride on magnets that lift them off the tracks but their held in place by side arms that rap around the tracks.
