You visit a local, independent insurance agent for the insurance and bond, and your city hall for the license.

The insurance is probably going to cost you $1500. The bond, well, it doesn't cover YOU, it only covers the city, or each client, and should cost you about $250. If you do the client one, you need one for EACH client. Bonds don't really do what people think they do. You'll LIKELY have to financially guarantee the bond amount to the bonding company.

find someone who is all of these and ask for proof. You will get their bonding agent, their place of license and whatever else they have and then just contact those places and do what they need you to do.
