Hi,
I am need to go on Short Term Disability but i dont know who to start with. Do i tell my manager about this or should i go to the HR.
Can some one guide me on how it is done usually?>

Go to your employer's Employee Assistance Plan if they have one. If not, go to HR.

The first person you see is your doctor to get a written note saying you need are are temporarily disabled and the diagnosis and for how long he wants you on disability. You need step is to take this note to HR and they will take it from there.

I would think the polite way to go around it is to see your manager, most people understand if you need time out for whatever reason.
