Pay attention in class and study, study, study.
Perhaps you could get a tutor if you need more help than you get in class.

First come to class everyday even if you dont want to, listen to the lesson and take good notes, do all the classwork and turn it in, particpate as often as possible, do all your homework, make sure to prepare for test and quizzies by studying. Continue until your grades are where you want them to be then you can lean off a little but continue to still do all of these things. MAke up any work that you missed if possible if not ask if there is anything like extra credit that you can do in place of that assignment.

Go over your notes and reading assignments each day (or evening).

Do not cram for tests as one doesn't retain the info to long term memory. Study over a longer period and what also helped me was to make up 'mock tests' for myself whether the teacher's testing style is multiple choice, fill-in-the-blank, true/false or essay. Practicing possible test questions can really help a lot.

Make note cards out of 4x6 index cards and separate them into piles as you learn the info. Though keep going over them until you know them by rote, forwards and backwards.

Perhaps join a study group of those that you trust to try to look at the key points of a discussion and for reinforcement.

If need be, ask your parents about getting you a reputable tutor or joining a tutoring center .

If you still have problems grasping the info, perhaps you can get a school evaluation to help with some kind of learning problem. (And if you are in high school, there should be a school social worker on staff, and he or she can talk to you about possibly setting up for school testing to be able to help you.)

The way to raise your grades is simply to pay attention. Write down homework assignments, take detailed notes, ask for help when you need it, and read. Read anything. Just not any form of comic books. Reading stimulates the brain.

do extra credit work - ask your teachers, i bet they have some extra credit assignments just waiting for such a request.

study. seriously - and not with the stereo on, or while listening to your ipod, or watching tv. ask some of your friends or classmates if they would like to form a study group. better yet, ask a really really smart class mate if they would be interested in helping you out.

make the extra effort on your regular homework assignments. neat handwriting, double spaced typed reports (use a larger font size and spell check too).

look for a tutor in the subjects you are lagging behind in. odds are, there are a few students that have this listed on your schools bulletin board, and probably for a reasonable fee.

ask your parents for help. they might be able to help you with problems you can't solve, and it might just make them softer on you if you can't manage to bring up the grades as much as you wanted to. parents tend to give A's for effort, if you know what i mean.

You can raise your grades by studing extra, listen to what the teacher says, and take neat notes.
Tips about note taking:everytime a teacher says something twice, its most likey to be on a test and dont doodle that much on your notes,it'll get you confused

Study hard which means stop watching television, playing computer games, and surfing the Internet. Your free time is focused on the subjects that you want really good grades in. The only free time you will have is summer time. Good luck.
