Common cold,
Cholera
Measles
Rabies
Hepatitis
AIDS
tuberculosis

typhoid
yellow fever

I would say:
Smallpox
Measles (it is very contagious)
Adenovirus (contagious)
Ebola
M. tuberculosis infection
HIV
Hepatitis
Influenza A
Strep pneunoniae
Mumps

not in that order mind. It depends on how you mean as well: most pathogenic, most contagious or highest mortality rate. Eg rabies: not many cases in the west, but almost all end up in death.
