I got a letter telling me about a FORM I need to fill out and it says forward to "my attention." Does that mean put the person's name on the letter I am sending?

yes, it means send it to whoever sent it to you

You would probably include a cover page of some sort. On it, just put "Attn:_______"
Put the person's name in the blank.
