It's not typically accepted that you salute unless both you and the other party are members of the military and they are an officer, or unless they are high government officials. As for using left hands, I can't say I've ever seen that happen.

The right handed hand shake is a carry over from the 1600 - 1700's in Europe. The open hand shake with the right hand was to prove that you did not carry a weapon (concealed knife or whatever), and that you came in peace.
As for the salute, I know absolutely nothing about it.

If both persons were in the military,then protocol would demand that the junior ranking person salute first,then when the higher ranking person lowers his/her salute,THEN you shake hands. If one is injured on the right hand,then the left hand may be used.

I have seen someone shake with their left hand. He had a deformed right hand, bent backwards. Some people were taken by surprise, but when they noticed why he offered to shake with the left, they happily accommodated him.
