Treatment of phenobarbital poisoning includes alkalinization of the urine to hasten excretion (because barbiturates are weak acids and are thus made more-soluble in alkaline urine). It is very critical to maintain the airway and provide oxygen, and also consider gastric lavage (within 24-48 hours).

Though barbiturates are CNS-depressants, note that CNS stimulants will NOT reverse effects, and may actually exacerbate them, so those are strictly contraindicated for treatment of toxicity.

First of all Diagnosis is important, And this is made on history and examination, though the blood level can be obainied too, on examination mainly we wuld se the CNS problems.....like unconsiousnes,or coma, constricted pupils, depressed breathing, confusion , hypotension etc..
managment should be accordingly

Maitiain the ABC,(airway breathing and circulation), gastric levage followed by activated charcoal, which prevents its furthur absorption, multiple doses should be admistered ! Alkalinization should be carried and IV fluid should be infused.......pluc diuresis should be done by giving diuretics in order to excrete it through kidneys ! and patient should be monitered carefully !
