Because she is insecure and has a low self-image. She was probably abused as a child and still thinks everything is her fault. It would be very hard to convince her otherwise.

Even if you took the guy out of the situation, she would need to go through a lot of therapy or at least deal with some hard realities.

Most people don't want to confront their demons, so they go on being miserable. So tell her your honest feelings on the matter and hope that she comes to realize it too. Good luck.
