Depending on what you can spend why not a vacation someone just the 2 of you. And if a week long vacation is to short or notice as of right now. Go somewhere for the weekend together, B&B or somewhere she likes. And to really top it off by her some jewelry to give her on the trip. Also write her a letter telling her how grateful you are to have her as your wife all these years.

The traditional gift for the twentieth anniversary is china. Something sweet you could do, is replace a piece of your wedding china that may have broken, or have your wedding picture put on a china plate that matches your wedding pattern with a lovely inscription.

I know you can book an advertisment at the movies. Take her to the movies and when she see's the advertisement on the screen seeing how much you love her and saying how much you have loved her during the past 20 years. She will spin out.
Otherwise you cane go for the traditional presents.

a medal? ok sorry bad joke, how about a nice scrap book kit, so she can go through old photos starting with your wedding or even dating and organize her memories of the two of you, and maybe a camera if she doesnt have one.
