Go to the store and buy some fortune cookies. Take out the fortune and substitute your message. Such as Confucius says "say yes to the next question you are asked". Then, go up to her hand her the cookie. Once she reads the your fortune, ask her out.
But my first thought was to tell you ask her straight up.

Draw it on a piece of paper and then cut it into a puzzle. Put the pieces into an envelope and hand them to her.
Write it on a balloon and give it to her.
Put it on a note with flowers on her car right before school ends.

well i just got asked to the prom and what my guy did was we went to a play with some friends and then he dropped everyone off and when he was dropping me off he asked me. so do something like that take her somewhere with mutual friends and then ask her =]
