You don't. Japanese don't ask that.
There are altneratives, though, like
genki desu ka?
"genki" is very difficult to translate, but basically means "energy", both physical and emotional. People are said to "have" or "be" genki if they have positive energy. This question means "are you genki?"
kigen dou desu ka?
"ki" means "feeling" and "gen" means "origin" or "home", so this is asking "what's your mood like?" or "what kind of mood are you in?"
kibun dou desu ka?
"bun" means "part" or "section." This is very similar to the above question, but this is more about emotional state than mood.
choushi dou desu ka?
"choushi" means "condition", as in "how's your health?"
The top question is the most common, but all of these questions are rarely actually used in Japanese.

Japanese doesn't really have a word for "How are you?"
o-genki desu ka ãå
æ°ã§ããwould be a literal translation, but it is almost never used as a greeting except on paper.
They are more likely to say
"o-hayo gozaimasu" ãã¯ãããããã¾ã- Good Morning
"konnichiwa" ããã«ã¡ã¯- Good Day
or "konbanwa" ããã°ãã¯- Good Evening
