You best bet is a water purifier. MSR (mountain safety research) make a good one.

dried food-dry soup mixes from the health food store work well, and don't weigh too much. also bring a fishing pole for food.

bug spray, winter hat, chocolate, are great to bring too.

There are as many answers to this question as there are people qualified to answer it. I suggest reading various books and then form your own opinion . The single most important thing is experience. Start off with a 2-3 day trip or 2, get a feel for the equipment and how it carries. when that feels good go for a week long trip or 3. After each of these trips you will change your gear or the way you pack it. It will become apparent quick that carrying 2/3 of your body weight is not the way to go

You will need to either pick a route that has water supply on it or prep the route and hide/bury water along the way. If you will have access to streams, ponds etc then the MSR water filter is the way to go, if you can not afford the weight or space then strain water through a bandanna and hit it with potable aqua tablets

Good Luck and Happy Trails

The most important thing is purified water. First of all, whenever possible, get your water from a running source or directly from a spring. Next, you should always purify it if possible. I recommend a water purification pump, but you should also take along some iodine or potassium pills (sporting good store) which can be used for purification. You can also boil the water if necessary, and always take canteens or other containers in case you have to travel a while without a water source.

Everything else you take depends on the terrain. You will need some sort of shelter, such as a tent, and the type would depend on the weather you expect to encounter. You will also need a good sleeping bag... I recommend the "mummy" style sleeping bags. Also, take clothes for every type of weather... it can change a lot in three weeks. On that note, you should take some type of emergency band radio to get updates.

Good maps, GPS, or a guide are also important, as well as a first aid kit. You'll want to take food high in calories, because you'll probably be burning a lot. You DEFINATELY NEED TO plan your trip in advance and let someone know where you'll be and when you'll be back, and it is a good idea to have a friend go with you. Also, if you can maintain communication with civilization, it would be a good idea, such as through a cell phone or CB radio.

Other good things to take: cooking utensils, multiple types of fire starters, etc. 3 weeks is a long time, and you will need a lot of gear to last you that long.
