I had a dreadful experience in one of Virginia's top country clubs, where a friend had booked me to stay for a weekend. I'd just spent a long time in New Zealand, where tipping isn't really expected, and didn't know my way around US etiquette very well, being British. I asked my friend to join me for dinner; it was a self-service buffet and the only thing the waitress did was open a bottle of wine.
After a lovely meal I went to the ladies' room and had to pass the door to the restaurant in order to catch up with my friend. The restaurant manager and the waitress were standing at the door waiting for me. I approached with a smile - I honestly thought that perhaps I'd left a handkerchief or something at the table. Full-on and without any preliminaries, the resturant manager said 'We were wondering why you didn't leave a gratuity?' I was very embarrassed and angry because this was happening in sight of my friend. So I thought OK, I'll embarrass you back ... and handed over my wallet and asked them how much they wanted. I thought that might actually shame them, but no, they took $20 without turning a hair.
But after I'd rung the hotel manager, I'm afraid that they didnt't have a job to go back to. Culture clash it may have been, but I still think it was sheer rudeness in how they handled it.

Generally yes - i usually give a 10% tip to the waiter/waitress when i pay the bill. However, if it is bad service then no i will not tip. Tips should reflect the service - if you give a tip to someone unworthy of a tip, you are defeating the object.
I know it is slightly different here in England than it is in America, however i believe it is polite to tip, but false to tip if it's undeserved.

In countries where it is customary, yes. That said, however, how much I tip depends completely on the service received. I will tip less than 5% if the server is rude, and over 30% for outstanding service. No tip at all could mean you forgot or are from a country where tipping is not practiced. A tiny tip makes the point clear, iun my opinion.

No, definitely not. TIP means To Insure Promptness. If a serving person is not attentive, or if they serve food that they should know is unacceptable or if their demeanor is inappropriate or if they don't know the answers to questions you pose about their food or wines, then they have not earned a tip. Also, remember that if you are a part of a large group, some restaurants automatically add a service charge, regardless of the service they provide. If you tip on top of that, you are paying twice for the same thing.

Yes, I always do.
10% minimum even if the service was poor. I know that it's not always the waiter/waitresses fault, and if that is the case, then I tip more. If the service is good, then 20% +.

Nope - it they suck, are rude, forget things and are just brutal, no tip. In cases of exceptional service I tip well. We had a dinner at a restaurant this weekend for a birthday and the total bill came to just under $400. We had 2 servers so my dad gave them each $50.00 each and I gave them and extra $20.00 each. They were great servers and I feel $70.00 each was a proper tip. Tips are not automatic but earned!

Usually the answer is yes, but if they don't help me with anything or they're rude, then no. I've never worked anywhere that I could recieve tips, so I'm not sure why some people expect a tip from me just for doing their job. If they are nice and helpful and they make my dining experience better, then yes, I do.

Yes, I do. I try to tip about 20%, unless the service is poor. Then, I tip about 10%. The servers work long hours and the extra money is helpful. I try to remember that some are working their way through college or are supporting families.

Yes, I leave a tip no matter how bad the service was. However, the size of the tip is directly proportionate to how good/bad the service was.
