To slowly and progressively remove something which was for some time considered common and good for the times.

It could be as easy as updating a filing system to be more efficient and run smoother, and slowly replacing and removing the old system which worked and has been used for years.

e.g.
A lot a school canteens have begun to phase out any snack foods which are considered to be unhealthy in a fight against the war of child obesity.
