Perhaps you are wanting to register a new birth, if so your doctor has the birth certificate and check the hospital. For your own birth certificate here is a link below that tells you all about it.

Umm you might want to be a little more specific and detailed on what you are asking . If you are talking about a birth certificate you need to go down to the health department . that is usually where they carry them and you will have to pay for it . they will have all the paperwork you will need to fill out .

If u are asking for an app to have your baby. If you have insurance,any type, even AHHCCS you need to find out where you can have your baby,remember doctors only deliver a certain hospitals,and where your insurance will pay.Hope that helps you sweetie ,cause for a while there you got everyone confused.

if you are talking about a birth certificate your local health department will have the forms you need to fill out. if you are in school you can obtain a copy from the school.

They don't apply to be born, they just pop out after 9 months or so.
