It means you keep doing the same thing over and over and being hurt repeatedly.

Its kind of like one of the definitions of insanity, "doing the same thing over and over again and expecting a different result". When you are a glutton for pain you keep going back to be hurt again even though you realize it will probably happen even though you don't have to.

Someone who keeps going back to the same thing, person, situation and knowing that they are going to get hurt over and over again. Thats just my thoughts on the phrase.
