Is it possible to convert or save a form made with Excel as a .doc-file?
When printed out as an .xls-file the form fits perfectly on letter-sized paper.

The simplest is to cut and paste your excel spreadsheet into a blank word document. It will have the same look as in excel, and you can modify it (but it will just be a word table, not a spreadsheet).
