All spots are caused by an excess of sebum in the hair folicle, if the sebum collects then it blocks the pore and becomes a blackhead, if this gets infected then it turns into pus and then you get a whitehead.
There are no magic solutions for spots, the best method to prevent them is to scrub or exfoliate the area so that any excess sebum is roughed away and the pores do not become blocked.
If you have persistan spots that become infected your GP can give you a prescription for an antibiotic called oxytetracycline this will help but it's not a permanent solution.
The best thing to keep spots at bay is a jolly good scrub everyday or so with an exfoliating mitt and a pH balanced soap/lotion, on your face a good scrubbing wash like Apri facial scrub will do the trick.

I'm a pharmacy technician and would say depending on the age of this person, it will probably be due to chemical and hormonal imbalances, although it's proven that the uv levels from the sun keeps skin breathing, opens pores and clears problem skin up (although it should be done safely), most people's backs get to see the sun very rarely, which is another cause for less clear skin.
Eat lots of fruit, drink plenty of fluids and keep it clear by cleaning it with soap free shower/ bath liquads, and wipe regularly with distilled witch hazel on some cotton wool. Should start to see improvements soon, see doctor if it's a persisting concern.

So many answers to that question. I would go see a Dr and maybe a second opinion, depending on how bad the prob is effecting you or who ever has this prob. That way it can then be treated and you will know the answer. There are many free and fee for less clinics that treat these even your county medical clinic fees are very low. Either they have acne... or you're doing too many drugs.

It could be that the clothes cause perspiration and the skin on the back can 't breathe causing blocked pores hence leading to spots.

used to have this problem. it was a combination of liver not funtioning properly(bad diet) & not getting enough air to it
scrub back every day & eat plenty of fruit !!!!
