Study Phonetics. Practice with a native speaker to learn the sounds properly.
Learn sentence structure. Many people still "think" in another language. English word order is othen reversed, particlarly with adjectives.
Prepositions also cause much difficulty. Learn the proper word for each relationship of ideas, direction of focus, dependency, etc. (Do not say "of" when you should say "from").
Verb forms need to be correct also. Learn the suffixes and conjugations and the main exceptions. (Do not say, "I am desire chocolate." ).

Practice makes perfect.

You need to go somewhere where you can talk English every single day.

Largely depends on where you live.

I would try to think in your mind using English too, that will help things a LOT in my own experience.

just meet some english friends and sit back and listen and join in on the conversation when the opportunity arises

Best way is get involved ina group or involve yourself with people who speak both English and whatever is your native language.
