I have. Multiple dog breeds, living in cages in warehouses or kennel blocks with several females to a block. These dogs are NOT pets, they have no family and their one and ONLY purpose in life is to reproduce more of their breed. I am surprised the breeder allowed me in the door.
Have you ever personally seen a similar situation?

Yes I have, and they are absolutely horrible. Even people that do not like dogs would want to take one home because they are in such bad condition. One of them had a pile of bodies behind one of the barns where the owner had disposed of all of the bodies of dogs that have died or that they "put to sleep" (which they did with a baseball bat). It was just so so sad, and I wanted to beat the s**t out of the owner for what they were putting the dogs through. I went to a "breeder sale" ignorantly thinking that it was actually a reputable breeder selling their breed stock. I couldn't believe that they let me see the kennels, and even had the audacity to say that they had cleaned the kennels up. There were over 6 inches of piled feces in spots, and there were 5-10 small/medium sized dogs in a 6 ft by 6 ft pens. It was horrible and I will never forget it, I just wish that there are stiffer penalties and fines for puppymills.

I did. The breeder had dozens of different breeds, mostly small dogs. They were kept in buildings, cages stacked floor to ceiling along all walls, 2-4 dogs in each cage. In all she said she had over 200 dogs. It was disgusting. Sad thing is our state does not care about animal cruelty cases and does nothing to shut down puppy mills. No one cares enough to do anything about them.

Yes. Don't think they aren't watched either. The laws in each state aren't strict enough to stop everyone of them. Humane Investigators know who and where they are. Eventually the mills will be raided, large numbers of dogs seized, millers charged with misdemeanors then start anew in another state...the cycle of abuse and neglect continues.
The "designer dog" craze feeds the puppy millers also to stay active. Puppy mills are not always large breeding facilities. The BYB's think no one notices them breeding a ***** every season and using her litter as a means of profit. Puppy mills happen everywhere and yes they will "let you in" to get your money.

Yes, I have, more times I care to remember!
I rescue dogs from the two largest mills in Tennessee, and being at it for the last seven years.
My very first time, I had nightmares for three months, and I would wake up from crying in my sleep. I've got a grip of myself, and accomplished many things that I'm proud of.
Sadly, it's a legal business in the USA, driven by people's greed.

Not in person, but at the local shelter I foster for in Missouri (AKA the puppy mill state), we get in a lot of seizures from puppy mills. The dogs come in looking just pathetic.

This is why I won't buy ANYTHING from pet stores. No leashes, no kibble, no grooming services. Apparently they have the same kind of mills for cats and ferrets and others. Anytime you buy from a pet store, you support the mills... you should definately report this experience. You can help stop it.

Ive never seen one, only pictures and it is heartbreaking. Yeah, they let you in, they arent ashamed of what they do. THey are dispicible excuses for human beings. I wish they were outlawed. Stuff like that breaks my heart. I do rescue for unwanted animals, so I see the pain every day.

Yes, and it made me sick! I called the ASPCA on them. Dogs are man's best friend, and they do not deserve that!
