My sisters cat has been puffing up her tail and moaning... but she was spayed long ago..

If the procedure was not done correctly and completely, yes although it is highly highly unlikely. I would be more concerned that a medical problem is causing her behavior...get her checked out by a vet.

Well spaying is a complete hysterectomy, so it would seem weird she would go into heat....

If I am correct a cat in heat screams loudly and drives you up the wall... moaning really isn't going into heat...

Of course I fixed my cat before she ever went into heat so what do I know but I have heard many horror stories..

Once spayed, a cat can no longer go into heat.

My male and female cats are both fixed, but they still mate. I don't know if she's "in heat", but she seems interested.

old habits die hard! It is just the beginning of "cat season".
She herself is not in heat, but is responding to "sounds" and pherimone "scents in the air" of others ...that probably are.

YES I HAVE MALE AND FEMALE CATS AND THEY STILL GET INTO HEAT EVERY HERE IN THERE.

Yes,if the surgery is not performed right or if something is missed.This is rare but it does happen
