Is there a dictionary that defines the abbreviations that eye doctors use in their medical record keeping?

I saw an eye doctors chart and its all abbreviations as regards his tests and his findings. I need a dictionary to figure it out and learn what its about. Anyone know one by author and title?

There are medical dictionaries, which probably include terminology and abbreviations used by all medical professionals. See websites below.

I am sure that Dorland's, Stedman's, or any other medical dictionary will contain these.
