She would not want to get to the airport and be denied travel. Also check with the health insurance company to determine if they would cover delivery if that would occur. Some have a restriction on the number of weeks along that they will pay for if outside the home area.

There are seat belt extenders. Also if she is over 7 months pregnant, may airlines need doctors permission for her to fly. You might want to check on that also.

All airlines are able to provide seatbelt extenders if necessary. However, I would not suggest flying often if you are pregnant for two reasons. One, because flying can expose the mother to infectious diseases from other passengers, and two because flying above FL180 (18,000ft MSL), you are more than 50% out of our atmosphere, and as such our bodies are actually exposed to higher levels of cosmic radiation (especially above FL360) and if she were to fly very often on extended trips, it could possibly affect the baby or birthing process.

Tell her they have belt extensions that would fit pregnant people. Then when you check in alert the people at the ticket counter or agents at the gate you'll need a seatbelt extension for your sister (tell them you sister's seat number as well). They'll make sure she has one before the plane takes off.

Airline seat belts fit over the hip, not the belly. She'll be fine; if not, they have belt extenders.

I would alert the airline and get a confimation from them that there will be a belt extender if necessary. This will give your sister peace of mind.
