When I left click and drag it back to the quick launch bar, it only creates a "copy" of the icon. The source icon still remains on the desktop. Desktop icons are not locked and auto arrange is not on.

Please reread the question. The showdesktop icon is already present on the desktop. I dragged it there by mistake. When I try to drag it back, it only creates a shortcut to show desk top. I wan't the original show desktop icon (which is now on the desktop) to be put back on the quick launch bar, not a COPY OF IT?

1. Open Notepad
2. Type the following exactly:

[Shell]
Command=2
IconFile=explorer.exe,3
[Taskbar]
Command=ToggleDesktop

3. Go to File and choose "Save As..."

4. Click the Desktop button along the left edge of the window. Under "Save as type" choose "All Files", then under "File name" type Show Desktop.scf

5. Click Save, then close Notepad.

6. Drag the new Show Desktop icon from your desktop into the quicklaunch toolbar.
