Jesus gave the true faith to the apostles. He gave them a mission and the sacraments to transmit the faith and his grace (Lk. 1:2; Jn. 20:21). If the faith you practice did not come through a direct line tracable back to the apostles, there is no garrantee that it is the true faith. However, Jesus promised to send the Advocate to lead the apostles in all truth (Jn. 14:26) and to protect his church from teaching anything false. So, only the one church founded by Jesus on the foundation of the apostles (Eph. 2:20) is garranteed to teach all truth (Mt. 16:18).

From the Catechism of the Catholic Church:

The Curch is apostolic becasue she is founded on the apostles.

Basically this means that over the past 2000 years the Bishops who are considered successors to the apostles continue to pass on the teachings of Jesus to the Apostles.
