In the "from:" area you need to write your name and your address or PO Box. It must be legible.

In the "To:" area:

First and Last name, his number
Address of the facility)

For example (for federal prison):

John Smith #368-63485
U.S.P. - Big Sandy
PO box 2068
Inez, Kentucky 41224

Most facilities have websites. If it is a federal prison, go to www.bop.gov find the facility on it and then go to "Mail" option.

If it is a state prison, go to the Department of Corrections of that state, find the facility and see how to address mail there.

Correspondence sent to inmates in prisons are addressed with the persons name and the full name of the correctional institution. The prison mail room sorts out the mail and delivers it.

Normally at least in the US, it will be the persons name, thier inmate number and the name and address of the prison.

Cell blocks change all the time, over night even, so normally that is not part of a mailing address

Just make sure his name is on the envelope as well as the letter, they do open up the letter and read it first before they give it to the prisoner.
