Cat's in the wild don't meow as often as their household counterparts and each household cat is different. The cats that do respond more have figured out that communicating helps to get their needs met. Our pets learn behavior by our behavior to their actions. I don't feel there is enough information in your question to give you an exact or even good answer but I do believe he/she has worked themselves in a routine of communicating with you.

Try changing your nightly routine and/or reaction to the meow for a bit and see he/she changes. You might be able to pinpoint the reason for her/his behavior.

It could be that's she hungry. Cats seem to have their own weird internal clock. Or perhaps she can hear your neighbour getting home at 12:30 from work every night etc. and is responding to that.

Lastly, I know someone who's cat did this as she got older and became senile. She would meow for 10 minutes at 6:00 every day.

It's habit now because you have been responding to that meow for years. Do you pet the cat when this happens, roll over and move him/her or get up and give them a treat. Whatever you do is reinforcing their behavior just as with a baby. Difference is a baby will grow out of it, the cat will not. So whatever you are doing, it needs to stop and so will the the midnite wake up call.

There is probably something outside that happens at that time every night. Perhaps a train whistle or a high frequency sound from some other source.

Mine does it 'cause he wants to play with me. He feels "you've slept long enough--play with me!!" because he's been able to catnap all day...
