Yes. The ONLY qualifier is that the service must have included long distance service. That can either be bundled with the service or paid separately. Land line service, mobile service and VOIP services all qualify as long as you paid for the service at any time between March, 2003 and July, 2006.

It can, but the person has to have paid the excise taxes on the bills. Some service providers do bundle long-distance service in with the contracts, but it would be wise to check with the service provider first.
