Tough one. Depends on your personal style. But don't approach from behind and make a sneak attack. Like a prey animal she'll run when the predator moves in from behind. Always approach from her line of sight for best results.
An added suggestion to help pick out the one most likely to say yes is this: Search the crowd for the girl already moving to the music but not on the dance floor. Even with friends she should be facing the dance crowd and not trying to talk to anyone. Look for a smile or a relaxed look on her face. Work to make eye contact. If she meets your eyes, looks away and then looks back at you or she keeps her eyes on you then your chances are pretty good.

I have relatively very less experience in this. What I found was just start dancing would be probably the best thing, if she is already dancing. Asking may be the respectful and would look civilized, but in my limited experience, i was told NO for asking where as there were no complains when I danced directly.

as a woman--If a dude walked up behind me in a club and started to dance or tried to touch me he very well may get some serious bodily harm done to him. May I suggest that you go up to the lady and say "hey beautiful,I saw you from across the room and wanted to know if you would like to dance??"--

A girl wants to be asked... but don't gyrate arond her or behind her. it embarasses her and she isn't impressed.
Be a gentleman and do it properly. Let her lead you onto the dance floor. Let her pick the spot.....
