I would suggest a small frame .45 with an Inside the Waist Band holster (IWB) I have a rather small frame, and can pull this off as long as I'm not wearing a wife beater or a tight shirt.
The Springfield XD is a rather amazing new weapon, incorporating Glock and Colt functionalities. Go to a range or a gun store, hold various guns and ask for their input. Go to a few online gun boards and ask about various weapons.
I will tell you this right now. YOU CANNOT GO WRONG WITH A GLOCK. I swear to god those guns are idiot proof. Get a Compact and a decent IWB holster.

If you are comfortable with carrying the 1911, I'd stick to that format. Look at one of the smaller Kimbers or at a Springfield Micro 1911. I have one with night sights and I really like it. While not as accurate as my full sized 1911s, I have no trouble keeping five shots in about 3.5 inches at 25 yards. This is easily good enough for defensive purposes as most defensive shootings occur at 7 yards. I've become a believe in night sights these days.

None of your chopped up mini guns are going to be as accurate, or as easy to control as a full size gun. The smallest semi-auto I would carry is the Glock Model 19 in 9mm, or the Glock model 23 in .40S&W. These are the "Compact models, which are 1/4" shorter in the butt than the full size models. The "baby Glocks" for me are way too small to handle efficently, and accuracy would suffer greatly.I would NEVER buy anything that small.

I'm in WA, and it's an open gun law state so we can carry with conceal permit. I carry a .22 Beretta Cougar whenever I go fishing or out at night. It's very accurate and small enough to fit in your back pocket. It was my fiance's back up gun when he was on duty, he has big hands too and it fits him perfect.

I like air-light 5 shot revolvers. They are light weight and fit in your pocket. Revolvers are more reliable than automatics. Just remember in cases of using a gun in self defense you are normally talking 5 feet or less. Almost anything is accurate within 5 feet.

I would recommend the taurus millenium pro. They range in caliber, but are highly efficient and are somewhat easy to conceal.
