Most of the pedestrian injuries and fatalities happen because someone fails to have the incite to look in all directions, not just left and right, but in front and behind them as well.
Look at it this way. An intersection has four directions to it. You can either look at it as left, right, ahead, and behind.
As for the fatalities, if the pedestrians don't pay more attention to where and when they take the step off the curb, then unfortunately, a lot of them get killed. If the people would just start reading the street signs and cross ONLY at marked crosswalks and not try to run across in the middle of the block, then there might just be a few less fatalities.
Then again you have to remember that it isn't just the pedestrians that cause this problem. Motorists have to take their share of the blame, too.
I've seen where people have been hurt by drunk drivers. This is something everyone, young or old, big or small, can all help to solve. If you see someone who is driving who just came out of a bar, they should be reported to the police and arrested. Any thing good to get him or them off the road fast.

Carelessness crossing the road.
When I've been in a car, most pedestrians seem to think the driver is telepathic and its a nightmare trying to second guess people who look poised to, and often actually do, dart out in front of an approaching vehicle.

mobile phones being used by drivers- so a delayed reaction when they see some one crossing the street- ot they are texting whilst driving (!!!!)
Or a pedestrian talking /txting on the phone whilst crossing it, not seeing the car that wont stop or the bicycle which cant stop and will crash into the road inorder to not knock over the pedestrian, and then in turn gets run over by the car.
naaaaa- its dink driving- kills the most every year by far- check out the police website, they bang on about it every summer and every christmas when the most drink-driving accidents occur.

Kitchen, is the most dangerous place in your house , followed by the bathroom.
I always walk around my kitchen , so that's a pedestrian area isn't it......
I'm going to guess, broken or uneven paving stones/cobbles that cause people to fall over......
M : )
And for fatalities in pedestrians, I'm going to guess, out of control car drivers(dangerous/drunk driving). are the cause of the most fatalities.

Alcohol. Drunk drivers and/or drunk pedestrians. I learned that in traffic school.
